A. Forms to be completed for Pine Grove School Registration
- Student Information Sheet
- Residency Registration Form
- Home Language Survey
- Migrant Education Family Survey

B. Documents to supply in order to complete registration
- 2 Current Proofs of Residency: Current MS Driver's License and Current Utility Bill
* the Utility Statement MUST show hour service address.
* both proofs must have the same address
* the 2 valid proofs MUST be on file before students can begin school.
- Immunization Form
* students entering 7th grade MUST have an updated Form 121 showing the booster shots.
- Birth Certificate
* must be on file before students can begin school.
- Social Security Card

C. If transferring from another school, verification of grade level and/or credits must be verified by the former school before admission into Pine Grove is complete.
